Background Information Disclosure Forms
A Background
Information Disclosure form must be completed by everyone 10 years old or
older who reside in the household where the child care will be provided. Anyone
who will have regular contact with the children will also need to complete
this form. This form is used to check criminal history.
Consent for Release of Information Form
A Consent for Release of Information form must be completed by everyone 10
years old or older. All substitutes or anyone who will have regular
contact with the day care children must also complete one of these
forms. This form is used to check whether or not there are any concerns
regarding child abuse or neglect.
Tuberculosis Test
You will
need to submit a copy of the results from a negative tuberculosis test. If
you have had a test in the last twelve months a copy of the results will
be acceptable. If
you test positive for tuberculosis and after having a chest x-ray are
determined to be free of tuberculosis, a statement to this effect from
your doctor is required.
This test may be done by your physician or your local county health
department.
Landlord
Permission to Operate a Child Care Business Form
All
applicants who rent their homes must have their landlord complete and sign
this form.
Regulatory Agency Approval/Acknowledgement to Operate Child Care Form
All
applicants who are either a foster parent or a State Licensed Child Care
Provider must submit this form to the regulating agency to be completed,
granting their approval for certified child care to be provided in the
home.
Class Requirements
To
achieve Level I, you will need to complete the 15-hour class entitled
Introduction to the Child Care Profession (Module A) and also the 8-hour
class entitled Fundament in addition to all other requirements. If you
provide care for families who receive county child care funding, your rate
of pay will be less at Level II.
You are responsible for finding out when this class is
offered and to register for it. The voucher you receive must be submitted
to Western Technical College when you register for the
class. You are also responsible for the cost of the required text

Continuing Education Hours
Certified providers are required to complete
five hours of continuing education per year.
The hours can be
completed by attending workshops at Family Resources, Western Technical College, or food program workshops. CPR will also be considered
continuing education. If you attend a workshop, be sure to ask for a bar
code or a certificate. This is required for documentation at your review.
The
continuing education hours will be monitored at recertification and spot
check visits. The year is based on the date listed on your
certificate. For example, if your certificate expires on June 4, 2004 you
would need to complete the five hours between June 3, 2002 and June 4,
2003. The other five hours would need to be completed by June 4, 2004.
SIDS: Sudden Infant Death Syndrome Training
All certified providers caring for children
under 12 months of age MUST complete a SIDS training before
becoming certified. This training
is included in the 15-hour Certification Class offered at Western
Wisconsin Technical College. Family Resources and Western Technical College offer the training on a regular basis.
Shaken Baby Syndrome Prevention Training
All certified providers caring for children
under the age of 5 MUST complete a Shaken Baby Syndrome Prevention training
before becoming certified. This training
is included in the 15-hour Certification Class offered at Western
Wisconsin Technical College. Family Resources and Western Technical College offer the training on a regular basis.
